Healthcare email is not a marketing channel. It is appointment reminders, pre-op instructions, results notifications, prescription updates, recall letters and portal invitations. When one of these fails to deliver, a patient misses an appointment, a screening is skipped, or someone does not learn that they need to come back in.

The address is usually captured once, at registration, often by a receptionist reading it off a form or typing it from a phone call. It is never checked again. Years later the patient has changed employer, changed provider, or abandoned the inbox entirely, and every message you send them silently fails. Nothing in the workflow tells the clinic that this is happening.

Undeliverable is not the same as ignored

The most expensive mistake in patient communication is assuming that a message not acted on was a message received and ignored. If the address is dead, the patient never had the chance. Verifying the contact list before a recall campaign or a screening invitation tells you which patients need a phone call or a letter instead, while there is still time to make it.

Registration is where the errors enter

A typo in a domain, a missing character, an autocorrected surname: any of these produces an address that will never work. How is patient contact data handled?

Do you verify anything other than the email address?
No. Mailthentic checks email addresses. It does not need names, dates of birth, record numbers or any other patient information, and you should not send them. Upload only the addresses you want checked. The less personal data leaves your systems, the simpler the governance question becomes.
Why can you not confirm a patient's Gmail or Outlook address?
Because Gmail, Google Workspace, Outlook and Microsoft 365 deliberately return a 250 OK even for mailboxes that do not exist, to prevent address harvesting. That means an SMTP check cannot confirm an individual mailbox there. We report this honestly with the SMTP response code and a confidence score rather than pretending to a certainty that no verifier has.
What should we do with addresses that come back unconfirmed?
Treat them as unreliable for anything that matters. In practice that means keeping a secondary channel for those patients: a phone call for the appointment reminder, a letter for the recall. The value of verification here is not that it fixes the address, it is that it tells you which patients you cannot safely reach by email.
What is greylisting and why does it matter?
Greylisting is when a receiving mail server returns a temporary deferral such as a 421, 450 or 451 instead of accepting or rejecting immediately. It is an anti-spam technique, and it is not a sign of a bad address. Mailthentic retries with a backoff rather than discarding the address, so a perfectly good patient contact is not thrown away because a server was cautious on the first attempt.
Can we verify addresses as patients register on the portal?
Yes. The real-time API can be called from your registration or portal signup form. It returns the status, the reason, the SMTP response code and a confidence score immediately, so an obviously broken address can be flagged before the account is created and the patient can correct it themselves.
